I am Mrs. Mattei, and I will be your child's school nurse.
Some important reminders: Students entering Grade 6 must show proof of immunization (Tdap and Meningococcal). Documentation should be submitted via email or placed in my mailbox.
Any medication that may be required during school hours or during a school-sponsored activity must have a completed healthcare provider medication authorization on file each school year. Students may be authorized to carry their inhaler and/or epinephrine autoinjector if their healthcare provider makes that indication on the required medication authorization form. Medications are stored in the school health office and must be brought to the school nurse by the parent/guardian along with completed medication authorization forms. Medications must be in pharmacy bottles (prescription) or in the original, unopened packaging (over-the-counter).
For those students interested in school-sponsored athletics (Grades 6-8): A sports physical is required. It is suggested to have your child's healthcare provider perform this physical during the annual well-child visit. Forms are available online. Sports physicals are valid for 365 days from the date of the exam. All sports physicals must be reviewed by the school district physician before participation can be approved.

Sick Day Guidance
When Should I Call My Child Out?
If your child has a temperature greater than 100 degrees and any other symptoms (cough, body aches, GI problem), please keep your child home and call the attendance line. When calling, please provide your child's name, grade and reason for absence. Please also leave a number where you can be contacted.
Your child needs to be fever-free for 24 hours—without the use of fever-reducing medication—before they can return to school.
If your child has been vomiting due to illness, they should be vomit-free for 24hrs and be able to keep food down prior to returning to school.
If you have questions, please contact the health office prior to dropping your child off at school.
Sports at ABIS
Any student participating in an intramural or interscholastic sports team is required to have a current NJSIAA sports history and physical examination.
The physical is valid for 365 days from the date of the physical examination.
The Pre-Participation history is to be completed by the parent/guardian. This form is to be reviewed by the examining provider at the time of the physical examination. All forms must be completed in ink and signed/dated where indicated by the parent/guardian.
ALL Health History Questions MUST BE ANSWERED. Any "YES" question must be explained in the area indicated. Unexplained "yes" questions may result in the sports physical not being approved and may delay the student athlete from starting the sports program.
The history form must be reviewed by the examining healthcare provider (keep the forms stapled). Completed physicals must be returned to Mrs. Mattei.
The health history update questionnaire is used to ensure the student athlete has not had a change in health status that may require medical provider review. The questions should be answered based on the student's health SINCE THE MOST RECENT SPORTS PHYSICAL EXAMINATION. Any "YES" answers need to be explained in the area provided.
No student athlete may participate in sports try-outs, practices or competitions unless the school district physician has reviewed and approved participation, based on the information provided in the sports physical.
